SESARAM's Project CAD Reaches Top Health Awards Finals

The Health Service of Madeira (SESARAM) has been shortlisted as a finalist in the 3rd Top Health Awards with its innovative project 'Cuidadores de Apoio ao Domicílio- CAD'. This project aims to improve the transition from hospital to home care.

The Health Service of the Autonomous Region of Madeira (SESARAM) has made its mark by progressing to the finals of the 3rd edition of the Top Health Awards. This achievement highlights the region's commitment to advancing healthcare solutions, particularly with the creative initiative known as 'Cuidadores de Apoio ao Domicílio- CAD'.

Selected from a pool of 110 entries, the project stood out in the 'Integration of Healthcare' category. The presentation to the jury was led by Márcia Assunção, the director of SESARAM's Social Service, underscoring the project's potential impact on healthcare delivery.

The CAD project seeks to bridge the gap between hospital and home care, ensuring a smooth transition for patients who remain hospitalized after clinical discharge. It aims to reduce delays in discharge by providing temporary home-based care tailored to individual needs.

During its pilot phase, the CAD project successfully reintegrated 48 individuals back into their natural environment, utilizing a model of personalized and humanized care. This project has already received national recognition, having won the 'Best Scientific Poster' at the 18th edition of the Good Practices in Health Award by the Portuguese Association for Hospital Development.

The Top Health Awards, organized by the Top Health Association, celebrate projects and individuals that significantly impact healthcare through innovative and collaborative approaches. The winners will be announced on March 25 in Lisbon.
